Sensible advice here from @dr.surdykapt. ✍️ Meaningful activities are often relinquished by clients for fear of making their condition worse. It is often thought that if exercise is painful, then it must be avoided. Fortunately, this is not always the case. So, when is it okay to have pain with exercise?
—
📌Is it tolerable? This is unique for every person. Whether you rate the pain as a 3/10 or a 5/10, it’s important to ask yourself if you’re comfortable with the level of discomfort associated with the exercise you’re performing. Remember, pain doesn’t always equal damage. ⤵️
—
📌Does it hinder sleep? If you’ve ever dealt with a shoulder injury, you know how difficult it can be to sleep on your affected arm. That’s not what this question is about, though. If you can normally sleep on your back with minimal symptoms, but your recent workout left your shoulder throbbing so badly that you can’t find any position of comfort, it might have been too much. ⤵️

📌Does it impact daily function? Once again, this is above and beyond its normal impact. Is your morning workout keeping you from gardening? Ascending stairs? Doing the laundry? If so, it may be necessary to back off a bit. ⤵️
—
📌Does it settle within 24 hours? Some individuals have little to no pain during exercises, but have an exacerbation of symptoms the next day. When you wake up in the morning, what are your baseline symptoms like? Let’s assume you go for a long run that same day with little to no pain, but wake up the next morning with far worse symptoms. It’s another indication that you might have pushed a bit too far. ⤵️

📌Is it impeding your goals? If you can squat 200lbs with a 3/10 pain and work up to 250lbs over the course of 3 months with that same 3/10 pain, that’s progress. If your performance is consistently improving or you’re achieving your goals without a worsening of symptoms, you’re likely in the clear. ⤵️

✍️If you can confidently provide the appropriate answers to the questions above, it is probably okay to exercise with some pain. Pain is complex and may fluctuate depending on the circumstances, so these are more of guidelines than rules.

🔑The symptoms of TOS are extremely diverse, which makes TOS hard to diagnosis at times and it can be just as difficult to treat. However, thinking about all the common problems associated & developing a plan to help fix each of them, is a great place to start!

◾️Here are 6️⃣ exercises that may help TOS: (& I included how each exercise will help below)
1️⃣Pec Ball Rollout on Wall with 1/2 Angel into Upper Trap Mobilization (Pec & Neck Mobility)
2️⃣Segmental Slides on Wall with Nerve Flossing (Nerve Mobility - typically median / ulnar nerve glides are a good place to start)
3️⃣Kneeling PVC Lat Mobilization Prayer Stretch (Thoracic Spine Mobility)
4️⃣Seated Wall Angels (Shoulder / Thoracic / Scapular Movement & Mobility)
5️⃣Behind the Head Snatch Grip OH Press facing wall (Shoulder Strength - there are so many others that are also helpful, but this is one of the ones I like to use because it helps keep the shoulder joint more open & it helps strengthen the shoulder into the overhead position, which I find always helpful with those who have TOS)
6️⃣Prone Row + External Rotation + OH Press + Angels (Posterior Shoulder / Rotator Cuff / Postural Strengthening)

⁉️Need an improvement in shoulder external rotation?
.
🔹Assessment. Take a look at how your external rotation is on each side. Are you limited on one side more than the other? If you feel like an improvement is needed, continue to the movement categories, otherwise if it is appropriate for the activity you desire, jump down to the strength category.

🔹Movement
.
1️⃣Assisted Shoulder ER. Use a light weight to help assist your shoulder into external rotation. Start by going to the floor and then move into a deficit.
.
2️⃣Floor sliders. Gravity is working in our favor to help maintain an ER position as we slide our arms overhead. Try and keep wrists and elbows down the whole time. (This exercise is also helpful for the thoracic spine).
.
3️⃣Wall slides. This requires more effort to maintain our shoulder in an ER position, so I therefore a progression to floor slides. By sitting with lets bent, we minimize low back extension during the movement.

🔹Strength

1️⃣Band ER (90/90). Build strength and control in desired ranges

2️⃣Face pull Y Press.

The problem is restrictive dieting and the obsession with the number on the bathroom scale. ⁠⠀
The problem is a perceived need that you must follow diet rules and a meal plan in order to succeed.⁠⠀

From my own personal experiences, I can tell you that (1)  a meal plan is an eating disorder typed up onto an 8.5" x 11” piece of paper and (2) diet rules are a recipe for anxiety. Anxiety will pull the plug on your attempts to improve your body composition.⁠⠀

I was a bloody mess when I was trying to weigh 118 pounds. I was unable to adhere to the strictness of my regime, and as a result, I felt chronically defeated. I could NEVER win.⁠⠀

I desperately needed a dietary and training approach I could maintain for the rest of my life. ⁠⠀
⁠⠀
Through experimentation with a flexible approach to intermittent fasting, I was able to restore my relationship with food. ⁠⠀

I'm just as human as you are. I want to be able to wear whatever I want without feeling insecure about my body, I want to look good naked for my man, I want to be able to move beautifully, I want to look & feel younger than my age, I want to be free of insecure thoughts so I can live without holding back, and I want my body to be strong and mobile. ⁠⠀

I had to kill the old Sara to become the new Sara. I don't particularly like the old Sara, so I don't talk about her much. She was insecure, she let men mistreat her, she starved herself, and she was painfully broken from muscle imbalances. ⁠⠀

I never would have thought I'd ever be able to do pull-ups, handstands, the splits or pole dancing. It still blows my mind that I learned these skills in my 40s. I realize it's hard for people to relate to me. But I want you to know I work my ass off so I can improve. That's the whole point: to improve.
